I, the undersigned parent or legal guardian of the child named above, give permission for my child to participate in all activities at Richard’s Virtual Reality Summer Camp. I understand that participation involves physical and virtual activities, and while safety precautions are taken, risks may exist.

I release and hold harmless Richard’s Virtual Reality Arcade, its staff, and affiliates from any liability or claims related to participation. I also give permission for the arcade to use photos or video of my child for promotional purposes unless I request otherwise in writing.
